How can I save the speech output to an .AAC sound file? 

With Narrator you can export audio presentations either as .AAC files or directly to iTunes.

There are two ways to export a presentation as an .AAC file:

  1. From the File menu, select Export as AAC....
  2. In the Toolbar, click the Export as AAC button.

After instructing Narrator to export as .AAC, a dialog box will ask which location you would like to save the .AAC file to. Select the desired location and click Save.

Once the file has exported, you will be able to find it in the location you chose in the Save As dialog. Note that each chapter is exported as it's own .AAC file.
